WhatRoute is based on a tool that was developed at an organization called CAIDA:

I spent about a year working for these folks while I was in San Diego. During that time, they were able to make a tool that would approximately figure out where a computer was located in the world. It is from that code that WhatRoute gets its geographical locations.

There is a third "old school" way to get this data, using the command line tool traceroute. That is of course where this capability first came to UNIX.
